When I've done it (more than once, sadly) it's usually been because my depth perception has been off due to one eye being farsighted and one eye being nearsighted. If at the time you were not overtired or distracted by a shiney something, perhaps an eye exam is warranted. But shiney is distracting enough to cause me to walk into things too, so hey. :D

This was actually one that most people who know both me and physics probably could have seen coming.
